How to Apply to the Doctor of Behavioral Health Program

The application process requires completing the formal application through the ASU Graduate School and completing supplemental application materials that will be sent directly to the DBH program.

Checklist for Application Overview

  1. Graduate College
    • Visit the Graduate College Web site at http://graduate.asu.edu/admissions/index.html
    • Review the “Graduate Admission Requirements” section
    • Carefully read the “How to Apply” section
    • Select the link for “Apply Now” or “Graduate Online Application”
  2. DBH Program Supplemental Application Materials
    • 3 reference letters (instructions below)
    • Copy of license(s) (instructions below)
  3. Interview (in-person or by telephone)
    We will interview each applicant as part of the evaluation process. Interviews may be conducted in-person or via telephone.

The DBH Admission Review Committee will begin reviewing applications in November 2009. Review of applicants will continue through March 2010. Admission decision announcements will be sent to students in April 2010.

Helpful Hints for the Graduate School Online Application

On the section “Academic Information” enter your GPA as requested, but you do NOT need to input any of the test scores listed on that page (GRE, GMAT, etc,).

When you see the page instructing you to select the degree, you will go through of series of drop-down menus:

1. For “Category” select “Biological Sciences, Health, and Wellness”
2. For “Campus” select “Downtown”
3. Select “Behavioral Health DBH”

DBH Supplemental Application Materials

Reference Letters (submit to DBH program address, fax, or e-mail listed below)

Three reference letters are required. Use the DBH reference form (Word or PDF) and:

  1. Fill in your name and address in the space provided.
  2. Check the program you are applying to.
  3. Give a copy of the reference form to each individual who has agreed to serve as a reference for you.
  4. Instruct the reference to mail, fax, or email the completed reference form to the DBH program.
  5. Examples of appropriate references include:
    • Your most recent employer or a supervisor who can assess your clinical performance and judgment in the area in which you wish to study.
    • Other professional (not a relative) such as a licensed behavioral health clinician, medical provider, or other health-care provider who is well acquainted with your professional practice and academic achievement.
